Used on a PI patient’s NP Exam visit per the Doctor’s recommendation for the patient to complete if they Doctor believes they sustained a head injury in the accident. The form will document their current state and show closed head injury symptoms they are experiencing. This form will be used at every Progressive Discovery to get an update of their symptoms if used on their NP Visit.
