After a patient has had their Recommendations for Care explained to them in detail they need to sign this form acknowledging they understand their financial responsibility for care in the office. This is applicable to ALL patients regardless of the care plan they are choosing to pursue.
Patient's will sign this form even if they don't commit to care at their ES1. The 100% staff member will sign this form too.
