After a patient has had their Recommendations for Care explained to them in detail they need to sign this Spanish form acknowledging they understand their financial responsibility for care in the office. It's 2 sided to be applicable to patients using our time of service options and/or their health insurance.
Patient's will sign this form even if they don't commit to care at their ES1. The 100% staff member will sign this form too.
