Updated 7/23. Given to patients on PD #3 to get an update on where they are at in achieving the Health Goals they established at visit #1. We ask the patient to share their testimonial/experience with 100% and chiropractic to inspire/encourage others. They can choose to write a testimonial or record a video testimonial on a future visit if they aren't prepared that day.
